Coverage of visits, testing and treatment is a contract between you, your employer, and the insurance company, not your healthcare provider.  As a courtesy, we verify benefits and file claims on your behalf, but encourage calling your company with questions about what is covered.  All co-pays are due at the time of service.  If your plan has a deductible, your policy's allowable amount is collected.  After insurance processing, responsibility for any uncovered expense rests with the patient.  If you do not have health insurance, ask about our self-pay discount.
